The fee is $150.00.
PPD/Tb test $10.00 (will need come back 48-72 hrs. for interpretation)
RPR/Syphilis blood test $10.00
Immunizations are additional. We have Td(Tetanus) $50.00 and MMR $75.00. Click here for Table A and other required immunizations and list of Health Departments. The Zoster and HPV vaccines are no longer required.
